Delayed Fat Transfer After Explant
Breast augmentation patients can remove their breast implants and replace them with fat transfer for excellent, aesthetic breast augmentation results without breast implants. My preference is to explant and immediately fat graft in the same procedure, but this is not always possible. Delayed fat transfer patients can still achieve excellent fat transfer to breast results, but some will need multiple transfers to achieve the best results. Each fat graft procedure will soften the past surgical scars and slowly lift tissue adhesions that may have developed after the initial breast implant removal. Even patients replacing large implants can achieve excellent results. The patient featured here developed painful capsular contracture and sought an immediate explant. Almost a year and half later she opted to undergo fat transfer to improve the shape of her breast and regain some of the volume she lost. She did not want a breast lift or other surgical revisions. She achieved good results from a single transfer, but opted for additional transfers to achieve better results. Ultimately she received four transfers with excellent aesthetic results. Not every patient will need this many transfers. The typical fat transfer patient will plan one or two transfers, but patients with significant scaring, tight envelopes, or with a desire for multiple cup size changes should plan for a minimum of two transfers. The photos above shows the final results of an explant with delayed fat transfer, whereas the slide below illustrates the process you may need to achieve these results.
Breast augmentation with breast implants is consistently the number one or number two most popular aesthetic surgery, and for good reason. Breast implants provide excellent results in the vast majority of patients. Breast implants, however, don’t last forever. Some patients develop capsular contracture, some implants rupture, and some patients decide implants no longer fit their lifestyle. If the time comes when you no longer want implants, fat transfer breast augmentation is an option to consider…..even if you explant first.
